Welcome To The Minto-Mapleton Family Health TeamThe Minto-Mapleton Family Health Team (FHT) consists of a group of health professionals working together with 8 primary care physicians and 4 nurse practitioners who have a combined patient roster of approx. 15,000 patients. While our corporate headquarters is located in Drayton, we also have offices in Palmerston, Clifford and Harriston and service the Minto and Mapleton communities.Using an interdisciplinary team approach to health promotion, prevention and chronic disease management, the FHT provides services promoting healthy lifestyle to improve your mental and physical well being.
