We serve more than one million residents in South Etobicoke, Mississauga, Oakville, Georgetown, Halton Hills and Milton All CCACs are funded by Ontario's Ministry of Health and Long-Term Care at no direct cost to clients. We are best suited to provide care to high-need clients so they can remain at home as long as possible. We are not the most appropriate option for all seniors or citizens. When you contact us, you will be assessed and either assigned to a case manager or given information and referred to alternative community services. A nurse or other licensed professional consults with the client and family, developing a personalized care plan, free of charge!
