The Hamilton Niagara Haldimand Brant Community Care Access Centre (HNHB CCAC) connects people in our region with the health and support services they need to: remain at home, avoid hospital admission, access support upon discharge from hospital and explore long-term care options.
It starts with a free phone call to 1-800-810-0000. Your call will be answered by a highly trained CCAC agent, who is a certified information and referral specialist. This means they have the skills and experience to understand your unique situation, working with you to identify the services that may help you.
You can receive a customized "My Health GPS" information package by email or Canada Post describing the services you select, including location, hours of operation, contact information, eligibility criteria and more. At any time, you can call back to ask more questions, or to enquire about other services.

Community Resource Service began as a result of the economic hardship that hit our community in the late 1980s. Originally called the Brantford Unemployed Help Centre, our organization started by providing support and advocacy to those affected by the economic downturn. As a local charity, Community Resource Service provides support to those in need and has developed programs to support individuals and families facing economic hardship, and for those needing support in finding employment. Programs, including Food Bank and Distribution Services, Employment Services, Advocacy and Christmas Baskets, have been part of our support to the community.
