Ontario's Ministry of Health and Long-Term Care (MOHLTC) introduced Family Health Teams in spring, 2005, designed to meet the primary health care needs within local communities and based on local patient population needs. This means that each Family Health Team may operate uniquely with programs and services based on the needs of their particular patient popoulation.
Family Health Teams are organizations that bring together different health professionals to provide the highest quality care for patients and are focused on chronic disease management and disease prevention and health promotion.
